Start out with a regular onesie. Cut off right above the curved hip sections of the onesie.
Fold under 1/4-1/2 inch. 1/4 inch works if you are leaving the hem raw. Use 1/2 inch if you are double folding the hem to finish it off. I was making mine as a shirt for daycare- and the life on these are limited, anyway- so I used a raw hem at 1/4 inch.
